Cardiac CT angiography (CTCA) uses high-resolution, ECG-gated CT with intravenous contrast to produce detailed three-dimensional images of the coronary arteries. CT-FFR extends this by assessing the functional significance of arterial narrowing without catheterisation.

How It's Performed

Heart rate optimisation via beta-blockers (target <65 bpm), sublingual nitrates for arterial dilation, intravenous contrast injection, and image acquisition taking under one minute. Total appointment duration: 1–2 hours. Patients must fast for four hours and avoid caffeine beforehand.

What Results Mean (CAD-RADS)

  • CAD-RADS 0–2 — low disease burden; medical management
  • CAD-RADS 3 (50–69% stenosis) — CT-FFR analysis to assess functional significance
  • CAD-RADS 4–5 (≥70% stenosis or occlusion) — typically prompts invasive angiography

  • Stable chest pain or breathlessness with intermediate coronary disease probability
  • Abnormal stress test results requiring anatomical clarification
  • New atrial fibrillation or heart failure requiring coronary exclusion
